29 CFR 1926.28(a):   The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions.   (a) On or about February 25, 2014 at a jobsite located at 244 Dockside Dr, Blue Springs Missouri-. Employees engaged in work that required the use of steel toe boots were exposed to serious crushing hazard, while walking and holding stable a 20 foot steel pipe, weighing 800-1000 pounds up a hill along side a forklift without proper personal protective equipment.

29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a). On or about Febuary 25, 2014 at a Worksite located at 244 Dockside Rd Blue Springs Missouri- Employees were walking and working under elevated pipe weighing 800-1000 pounds. Employees were not protected from overhead hazards by the use of hard hats.

29 CFR 1926.602(c)(1)(vii):   Unauthorized persons were allowed to ride on a powered industrial truck:   (a) On or about February 25, 2014 at worksite 244 Dockside Rd in Blue Springs Missouri-. Employees (two) were exposed to life threatening struck-by /crushing injuries, in that he was observed riding on the access steps of a JLG Gradall G6-42a power industrial truck,  as it traveled approximately one half mile down the side of a hill.
