29 CFR 1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:  (a) Smith College Campus House Northampton, MA: On or about February 26, 2014, an employee was using power tools that were plugged into a Defiant brand RPT which was powered through an extension cord, which is contrary to the instructions printed on the device and therefore not in accordance with this subpart.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Smith College Campus House Northampton, MA: On or about February 26, 2014, an employee was observed building a mechanical screen at the edge of this roof more than 16 FT above the ground below without any fall protection, which is not in accordance with this subpart.
