29 CFR 1910.146(k)(1): An employer who designates rescue and emergency services, pursuant to paragraph (d)(9)of this section shall evaluate the prospective rescuer's ability by the criteria set in this section, (k)(1)(i) - (k)(1)(v). A.) Sharon Coating LLC - On or about 2/27/14, the employer had not evaluated the rescuer's ability to perform a confined space rescue from all of the confined spaces in the facility. In addition, the rescue group had not evaluated and accessed the confined spaces to determine their confined space rescue capabilities and needs. Personnel enter confined spaces that included, but were not limited to, hot bridle and threading furnace.

Sharon Coating LLC - On or about 2/27/14, employees worked on equipment that included but was not limited to, furnaces. The employees working on the equipment had not received authorized employee lockout/tagout training.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) Sharon Coating, LLC - On or about 2/27/14, employees were performing servicing and maintenance on machinery/equipment including, but not limited to, the furnaces. Servicing and maintenance activities included but were not limited to, repairs, preventative maintenance, removing sludge and threading the furnace. The employees entering the furnace and performing the servicing and maintenance work did not apply lockout devices to the furnaces energy isolating devices.

29 CFR 1910.147(f)(3): When servicing and/or maintenance was performed by a crew, craft, department or other group, the employer did not ensure that employee(s) utilized a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device: A.) Sharon Coating, LLC - On or about 2/27/14, employees were performing servicing and maintenance on equipment/machinery including, but not limited to, the furnaces. The equipment was locked out by maintenance personnel but then it was entered by production employees to perform work. The production employees did not have locks applied. Production personnel performed the work in the equipment without having their own lockout device affixed to the energy isolation devices and/or group lockout devices. Servicing and maintenance activities included but were not limited to, repairs, preventative maintenance, removing sludge and threading the furnace.
