29 CFR 1910.25(d)(1)(x): Portable wood ladder(s) were not inspected frequently and those which had developed defects were not withdrawn from service for repair or destruction and tagged or marked as "Dangerous, Do Not Use":  (a)  Warehouse, Adjacent to the Freight Elevator Door: On or about 2/27/2014, the eight foot portable wood ladder had not been inspected and was being used by an employee when it had defects. It should have been tagged, marked for do not use and removed for repair or destruction.    Abatement Note:  An inspection program needs to be established for all wood and metal ladders and metal step type ladders.
