29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Roof Level: Employee without any means of fall protection while on top of building was exposed to a fall of approximately 22 feet on to an asaphalt driveway, on or about 12/12/2013.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid.  a) Roof Level: Employees used a ladder that was not secured nor did it extend at least three feet above the landing, to access a flat roof  being exposed to a fall of approximately 22 feet on to an asaphalt driveway, on or about 12/12/2013.
