29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:  On or about February 25, 2014- Employees were exposed to multiple fan blades which had holes in the guard greater than one half inch on the fan guard on the inside of the building and the fan guard on the outside of the building.   Abatement certification for this violation is required.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:  On or about February 25, 2014- Employees were exposed to serious injuries to the fingers and hands, in that the Kicker Chain on the Convey located 24 Inches above the floor was not enclosed and guarded.   Abatement certification for this item is required.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:   On or about February 25, 2014, at the McBride Sawmill Facility, employees were exposed to serious injuries due to electrical shock, fire, or burns, in that the metal electrical insulation on the saw mill equipment had pulled away at both ends of the metal raceway.               Abatement certification for this item is required.
