Worksite located at 4719 SW Raintree Drive, Lee's Summit, Missouri- Employee was observed working on a platform suspended by a forklift in which the platform was not provided with any means of guardrailing and thus exposing the employee to a fall hazard.

Worksite located at 4719 SW Raintree Drive, Lee's Summit, Missouri- Employee was exposed to a fall hazard in that the Lull front-end loader specifically states that it is not intended to lift personnel and was not designed by the manufacture for such use.

Worksite located at 4719 SW Raintree Drive, Lee's Summit, Missouri- Employees were exposed to fall hazards in that the hole leading to the basement area was not covered, nor was there any guardrails provided around the hole to prevent falling.
