29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.     (a)  Front of House - On or about February 19, 2014 employees were exposed to a falling object hazard while working beneath other employees. Employees were passing lumber and roofing material over other employees.

29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway shall be at least 18 inches (46 cm) wide.     (a)  Right Side of House - On or about February 19, 2014 employees were exposed to a fall hazard while working on a wooden scaffold with a walkway less than 18 inches wide.  The scaffold walkway was only 9 inches wide.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.     (a)    Front Roof - On or about February 19, 2014 an employee was exposed to a fall hazard while working on a steep roof 25 feet above the ground with no guardrail system or personal fall arrest system or safety net system. The roof pitch was 8/12; 12/12 and 16/12.     (b)   Right Roof - On or about February 19, 2014 an employee was exposed to a fall hazard while working on a steep roof 18 feet above the ground with no guardrail system or personal fall arrest system or safety net system. The roof pitch was 8/12; 12/12 and 16/12.        (c)    Right Roof - On or about February 19, 2014 two employees were exposed to a fall hazard while working on a steep roof 25 feet above the ground with no guardrail system or personal fall arrest system or safety net system. The roof pitch was 8/12; 12/12 and 16/12.        (d)   Right Roof - On or about February 19, 2014 two employees were exposed to a fall hazard while working on a steep roof 22 feet above the ground with no guardrail system or personal fall arrest system or safety net system. The roof pitch was 8/12; 12/12 and 16/12.
