29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Plating Department:  Employees were not required to use personal protective equipment while hand dipping parts into plating solutions: a) Nickel Plating Line-an employee who was dipping parts into the nickel plating solutions was not wearing hand and eye protection. b) Chrome Plating Line - an employee who was dipping parts into the chrome plating solutions was not wearing hand, eye, and body protection.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   Production Area:   Employer did not include in the employee training an explanation of the new label format and Safety Data Sheet (SDS) format and how to effectively access the appropriate hazard information on the updated labels and SDS's for employees who were exposed to various plating solutions such as nickel carbonate, nickel sulfate, sodium copper cyanide, and chromic acid.
