OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  Rig 80: On or about February 26, 2014 the employer did not ensure that anchor capacities are verified prior to attaching guywires to the anchors, that the verification is less than 24 months old and that the anchors spacing and capacity is suitable for the mast guying pattern and anticipated loading.  This condition exposed employees to struck-by hazards from the derrick of the rig.
