29 CFR 1910.23(a)(5): Infrequently used pit or trap door floor opening(s) with the cover(s) removed were not constantly attended by someone or protected on all exposed sides by removable standard railings:    (a)  HTI Truck Shop Service Pit - On or about February 27, 2014, employees working on Truck # 1217, which was parked over the 72 foot long pit, were exposed to falling into the 48 1/2 to 52 inch deep by 3 foot wide service pit due to the absence of covers or removable standard railings to guard the 22 feet of open pit behind the truck and the 4.5 feet ahead of the truck.

29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:    (a)  HTI Truck Shop, between Service Bays #2 and #3 (from West end) - On or about February 27, 2014, employees using the Baldor 1/3 Horsepower, 3600 RPM, Pedestal Grinder, S/N W9-89, were exposed to injury from the 4 3/4 inch diameter by 1 inch thick abrasive wheel due to the absence of a spindle end cover on the right side, when facing grinder.

29 CFR 1910.304(g)(6)(vi): Exposed noncurrent-carrying metal parts of cord- and plug-connected equipment that may become energized shall be grounded under any of the following conditions:    (a)  HTI Truck Shop, West of Service Pit, between Bays #2 & #3 - On or about February 27, 2014, employees using the portable NAPA 85-1250 Battery Charger and Starter were exposed to electric shock due to the absence of a grounding prong on the male attachment plug on the charger's cord.

(a)  Office of Howard Transportation, Inc., 107 Nehi Road, Ellisville, Mississippi - On or about February 27, 2014 the OSHA 300-A Annual Summary forms for 2009, 2010 & 2011 were not accurate, in that:  (1) the annual average employment totals were not properly averaged, and  (2) the annual total hours worked by all employees were simple estimates, not actual hours.
