29 CFR 1926.453(b)(2)(v): A body harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about March 3, 2014, employee(s) were exposed to a fall approximately 12 feet in height to the paved ground level below, while using a Genie lift to apply vinyl graphics on the buildings exterior wall without wearing fall protection.

29 CFR 1926.452(n)(3): Ladders used to support step, platform, and trestle ladder scaffolds were not placed, fastened, or equipped with devices to prevent slipping:  On or about March 3, 2014, employee was exposed to a fall of approximately 7 feet to ground level below, from standing on a scaffold that was not secured at either end of the working platform to prevent slippage out of ladder rungs while installing vinyl graphics to the exterior wall of the building.
