29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood whcih completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    a.  On or about February 26th, 2014, at the above addressed job site employees were exposed to an amputation hazard while operating a table saw that was not guarded.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     a.  On or about February 26th, 2014, at the above addressed job site employees were exposed to an approximate 12 foot fall when using an extension ladder which did not extend at least 3 feet above the upper landing surface while gaining access to the second story.
