29 CFR 1926.502(d)(17):   The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  a) Roof of the house located at Lot # 9, Walnut Grove, 278 Walnut Path SW, Marietta, GA:  The employer allowed an employee to wear a body harness with the attachment point of the belt not in the center of the wearer's back near shoulder level, or above the wearer's head while installing 1/2" OSB Plywood on a 8:12 pitched roof, exposing the employee to fall hazards of approximately 14 feet 6 inches.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) Roof of the house located at Lot # 9, Walnut Grove, 278 Walnut Path SW, Marietta, GA: Employees using Pneumatic Hitachi Nail Gun, Model: NR 83A3 (S) were not protected from eye injuries while installing 1/2" OSB Plywood on the roof.  Ezequiel Gonzalez dba Gonzalez and Sanchez Construction, LLC was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.102(a)(1) which was contained in OSHA Inspection Number 947994, Citation #1, Item #1, issued on 01/27/2014 with respect to a work place located at 214 Avery Street Marietta, GA 30060 and became a final order on 02/06/2014.
