29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) At 3006 McKinley Dr Opelika, Alabama: On or about March 5, 2014 and at times prior; the employer failed to ensure employees involved in roofing activities on a high slope roof were using fall protection as per the regulations exposing them to a fall hazard of 19.7 feet to the ground below. b) At 3006 McKinley Dr Opelika, Alabama: On or about March 5, 2014 and at times prior; the employer failed to provide fall protection for employees standing on top of the first level roof to gain access to the ladder exposing them to a fall hazard of 19.7 feet to the ground below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 3006 McKinley Dr Opelika, Alabama: On or about March 5, 2014 and at times prior; the employer failed to ensure a ladder used on site for employees to gain access to the first level roof was extended at least three feet above the landing area exposing them to a fall hazard of 9.9 feet to the ground below.
