On or about 3/6/2014 employees at the Arrow Head Trail construction project were exposed to approximately 12' falls. Employees were engaged in carpentry activities without fall protection.

On or about 3/6/2014 employees at the Arrowhead Trail construction project were not provided safe means of access to their work area. Employees entrance to the work area had a break in elevation that exceeded 19 inches.

On or about 3/6/2014 employees at the Arrowhead Trail construction project were not provided safe access to their work area. Employees ladder did not extend at least three feet above the landing.
