29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).   On or about 3/6/2014 employees at the Arrow Head Trail construction project were exposed to approximately 12' falls.  Employees were engaged in carpentry activities without fall protection.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided.  On or about 3/6/2014 employees at the Arrowhead Trail construction project were not provided safe means of access to their work area.  Employees entrance to the work area had a break in elevation that exceeded 19 inches.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  On or about 3/6/2014 employees at the Arrowhead Trail construction project were not provided safe access to their work area.  Employees ladder did not extend at least three feet above the landing.
