Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. a. In the south west corner apartment of building C where employees were using a portable heater to warm the area, a 2-gang square metal electrical box with outlets was attached to the end of an extension cord, exposing the employees to an electrical contact hazard.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. In the south west corner apartment of building C where employees were using a portable heater to warm the area, the heater electrical cord was damaged with the outer insulation jacket torn away, exposing the employees to an electrical contact hazard. b. In the south west corner apartment of building C where employees were using a portable heater to warm the area, the extension cord used to provide power for the heater was damaged in two areas through the outer insulation jacked, exposing the employees to an electrical contact hazard.
