29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about March 7, 2014, at a residential structure located at, or near 149 The Woods, Woodlands Edge, Little Rock, Arkansas 72221, the employer did not ensure an employee framing a residential structure was properly protected by the use of safety glasses while using the pneumatic nail gun. The employee was observed/exposed to eye impalement hazards.

Employees were observed/exposed to fall hazards of greater than 12 feet to unfinished ground below.
