29 CFR 1926.502(f)(1)(i): When no mechanical equipment was being used the warning line was not erected at least 6 feet (1.8m) from the roof edge:    (a) Baresville Elementary School Roof - The warning line was not maintained at least six feet from the roof edge.  Employees worked on the roof to install the multi layered built-up roof system and were exposed to a fall hazard of 12 feet from the eve to the ground on or about March 7, 2014.    No abatement certification or documentation required.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:  (a) Baresville Elementary School Roof - The point of access was not connected to the work area by an access path formed by two warning lines.  Employees worked on the roof to install the multi layered built-up roof system and were exposed to a fall hazard of 12 feet from the eve to the ground on or about March 7, 2014.    (b) Baresville Elementary School Roof - The material handling area was not connected to the work area by an access path formed by two warning lines.  Employees worked on the roof to install the multi layered built-up roof system and were exposed to a fall hazard of 12 feet from the eve to the ground on or about March 7, 2014. 29 CFR 1910.1200(h)(3)(iv):   The employer did not produce a hazard communication program that included an explanation of the labels received on shipped containers and the workplace labeling system used by their employer and the safety data sheet, including the order of information and how the employee could obtain and use the appropriate hazard information:  (a) Baresville Elementary School Roof  - The employer did not provide employees with training on the revised hazard communication standard (HCS 2012) label elements and the new format and type of information on each section of the safety data sheets (SDSs) to ensure that when employees begin to see HCS 2012 compliant labels and SDSs in their workplace, employees understand how to use the labels and SDSs and access the information effectively on or about March 7, 2014.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
