29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  a. Site- Workers were not protected from fire hazards where the controlling contractor failed to ensure that fire extinguishers were available when combustible fuel was stored next to an operating portable kerosene heater. The combustible fuel and heater was located in a temporary room with walls made of non-fire retardant plastic sheeting.

Site- The controlling contractor failed to ensure that workers were protected from trip, laceration, and trip hazards where construction debris was not picked up on a regular basis. Each level inside the tower has metal floor grating where small pieces of sheet metal and steel plate was left on the floor and could pass through the floor grating striking workers on lower working levels.
