29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, were not guarded.    a) Jobsite: Breakdown Room - On or about 2/21/14, shop employees who operated the Two (2) Person Pallet Dismantler within ten (10) inches of the unguarded saw blade were exposed to amputation hazards while dismantling and recycling pallets.  Model and serial number of the pallet dismantler is unknown.
