29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about 3/6/2014, at the above addressed jobsite, employees were applying roofing paper on a residential 5:12 pitch roof approximately 28 feet from ground level and did not attach the lifeline to the d-ring on their harness exposing them to fall hazards.
