29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a. On or about February 3, 2014, employees for SQC, Inc. were working off of scaffold on the east and south side of the residence to put plastic on windows. The scaffold was not fully planked, exposing the employees to falls of 18 feet.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: a. On or about February 3, 2014, an employee for SQC, putting plastic over the windows, did not have guard rails on the 3 level of the scaffold on the east side of the residence, exposing the employee to a 18 feet fall. b. On or about February 3, 2014, an employee for SQC, putting plastic over the windows, did not have guard rails on the 3 level of the scaffold on the south side of the residence, exposing the employee to an 18 feet fall.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: a. On or about February 3, 2014, the employee doing stucco work on the third floor did not use fall protection to prevent a 32 feet fall hazard.
