(b) Worksite, Building # 3, south side of second story - An employee installing roof rafters with a Grip Rite pneumatic nail gun, Model # GRTFW 83 / Serial # 11051518, was not provided with; or using any form of eye protection on, or about March 10, 2014. (c) Worksite, Building # 3, second story - Employees installing roof rafters were engaged in hoisting operations using a National Crane Boom Truck, Model # 900A / Serial # 293213, and were not provided with; or using protective helmets on, or about March 10, 2014. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.

Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.

The employees did not recognize the fall hazards; were unaware of the procedures to minimize them; and were not provided with fall protection training on, or about March 10, 2014. Note: In addition to abatement certification, the employer is required to submit abatement documentation for this violation in accordance with 29 CFR 1903.19.
