29 CFR 1910.24(c): "Stair strength." Fixed stairways shall be designed and constructed to carry a load of five times the normal live load anticipated but never of less strength than to carry safely a moving concentrated load of 1,000 pounds. a. On or about March 10, 2014, in the work area, the stairs going to the lunchroom had three stairs with the stair tread nose broken off exposing the employees to slip and fall hazards.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. Ironing Area - On or about March 10, 2014, the employer did not provide a guard to protect employees from coming in contact with ingoing nip points on the Ironer #1 Machine. Employees were exposed to fractures and contusions. b. Ironer Area - On or about March 10, 2014, the employer did not provide a guard to protect employees from coming in contact with ingoing nip points on the Ironer #2 Machine. Employees were exposed to fractures and contusions. c. Ironer Area - On or about March 10, 2014, the employer did not provide a guard to protect employees from coming in contact with the ingoing nip points on the feed and outgoing sides of the Spreader on Ironer #2. Employees were exposed to fractures and contusions.
