29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:  On or about March 6, 2014, at the above addressed jobsite, the employer did not ensure the employee working in the Genie Aerial lift had access to fall protection, to prevent him from falling out or being ejected from the basket.  NOTE: Certification of abatement is required for this item.
