29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    (a) Location - 130 South Main St., Middleton, MA  On or about 03/11/14, an employee was working from a carpenter bracket scaffold greater than 10 feet above a lower level without a means of fall protection.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.   (a) Location - 130 South Main St., Middleton, MA On or about 03/11/14, a portable ladder that was damaged was not withdrawn from service.
