29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs:  (a) At Allied Manufacturing Company on or about May 21, 2014 stairs leading to the drywall mixer work platform were not even in rise heights.  The rise varied in height between 6.5 inches and 10 inches.  (b) At Allied Manufacturing Company on or about May 21, 2014 stairs leading to the drywall mixer work platform did not have uniform treads.  The depth of the dreads varied from 6 inches to 11 inches.  (c) At Allied Manufacturing Company on or about May 21, 2014 stairs leading to the drywall packaging platform did not have uniform treads.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:  (a) At Allied Manufacturing Company on or about May 21, 2014 a set of steps leading to the work platform for the drywall mud mixer did not have railings on both sides of the steps.  (b) At Allied Manufacturing Company on or about May 21, 2014 a set of steps leading to the work platform for the drywall mud mixer did not have both a top rail and a midrail.  NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. 29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  (a)  At Allied Manufacturing Company on or about March 5, 2014 the employer did not perform an evaluation of the possible air contaminants such as but no limited to silica, respirable dust, and total dust to determine the type of respiratory protection and other personal protective equipment necessary while doing batch mixing of gypsum compounds.       (1)  On or about March 5, 2014, an employee doing operating a batch mixer was exposed to an 8 hour time weighted average (TWA) level of 8.50 mg/m3 of respirable dust, which is 1.691 times the OSHA permissible exposure level (PEL) of 5 mg/m3.

On March 5, 2014 a worker was sampled and found to be exposed at a level of 8.5 mg/m3 which is 1.691 % of the PEL.
