29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c).   (a) Parking Lot, Rear:  An employee inside a 5 foot deep by 4 1/2 foot wide and 20 foot long excavation with thawing, cracking and spalling Type B soil was not protected from cave-ins while using a hand shovel to uncover a pipe, observed on or about 3/10/14.   (b) Parking Lot, Rear - An employee was working next to an unprotected 18 foot vertical wall of an excavation while working on an 8x9 manhole box, exposing the employee to a potential cave-in, observed on or about 3/20/14.
