Site - On or about March 6, 2014, an employee assigned to work and retrieve materials from an overhead crane was not wearing a hard hat and was exposed to falling or flying objects.

Site - On or about March 6, 2014, fall protection was not utilized by an employee engaged in roofing activities and exposed to falls of over 79 feet.
