29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  At the job site,  651 S. Ohio Street, Salina, Kansas, employees engaged in masonry activities from a tubular welded frame scaffold were exposed to fall hazards of approximately 14 feet from the scaffold decking to the ground below in that guard railing was not installed at the east end of the working level.

29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:  At the job site,  651 S. Ohio Street, Salina, Kansas, employees engaged in masonry activities from a tubular welded frame scaffold were exposed to fall hazards of approximately 14 feet from the scaffold decking to the ground below in that mid-rails were not installed along the back side of the material level.
