29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by the use of a guardrail system, safety net system, personal fall arrest system, warning line system, or a safety monitoring system:   8700 Manchester Road: Ten employees working on a roof of a Fly shop did not use any forms of fall protection systems such as, but not limited to, guardrail systems, safety net systems, personal fall arrest system properly tied to a structure, warning line systems or a safety monitoring system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access:   8307 Manchester Road, Brentwood: Employees used a Werner extension ladder which did not extend a minimum 3 feet above the landing surface to gain access to a roof in the rear of the building.
