29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment had been performed through a written certification that identified the workplace evaluated; the person certifying that the evaluation had been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  Edge Trimmer Position-    On or about March 11, 2014 the employer failed to perform a hazard assessment and written certification for the use of protective eye glasses and/or a face shield for this workplace position.  The employer was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 927712, Citation 1, Item 2, and was affirmed as a final order on September 24, 2013, with respect to a workplace located at 48 Atlantic Avenue in Brewer, Maine.

29 CFR 1904.32(b)(3): A company executive did not certify that he or she has examined the OSHA 300 Log and that he or she reasonably believed, based on his or her knowledge of the process by which the information was recorded, that the annual summary was correct and complete.   Plant wide-     On or about March 11, 2014 the company executive had neither signed the form nor fully completed the posted OSHA 300 Summary in that the annual number of employees and total hours worked had been left blank.

29 CFR 1910.134(c)(2)(i): An employer provided respirators at the request of employees or permit employees to use their own respirators, if the employer determined that such respirator use did not in itself create a hazard. If the employer determined that any voluntary respirator use is permissible, then employer had to provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard")  Plant wide-   On or about march 11, 2014 the employer allowed the edge trimmer to wear a single use disposable respirator without providing to the employee the information contained in Appendix D.
