29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  (a) Building, Southwest Facade - On or about March 11, 2014 three employees of Mr. Juan F. Arellano Becerra were exposed to fall hazard when working on plastering activities and supervising from a scaffold that was not inspected by a competent person at the beginning of the work shift.

(a) Building, Southwest Facade - On or about March 11, 2014 two employees were exposed to fall hazard when working on plastering activities from a scaffold that did not have a midrail in the back of the bracket's working platform and did not have a guardrail on the side of the bracket's working platform.
