29 CFR 1910.215(a)(1): Abrasive wheels used on grinding machinery were not used with safety guards:    (a) On or about March 6th, 2014, employees were exposed to being struck by flying objects and/or possible finger amputations while working with and around two bench grinders that lacked adequate guarding in that one had completely exposed grinding wheels with no peripheral or side guards as well as unguarded vertical belts and pulleys.
