(c) Philo Insulation & Fireproofing Co. - The employer did not establish or implement procedures to ensure that respirators were cleaned, disinfected and stored in a sanitary manner as required by paragraph 1910.134(c)(1)(v) after spraying diphenylmethane diisocyanate (MDI), on or about March 14, 2014.  (d) Philo Insulation & Fireproofing Co. -The employer did not establish or implement procedures to ensure that employees were medically evaluated as required by paragraph 1910.134(c)(1)(ii) before using respirators for spraying diphenylmethane diisocyanate (MDI), on or about March 14, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
