Certification is to include, but not be limited to, the name of the operator, the date of the training or the evaluation, the name of the person providing the training or evaluation, etc.  a- Epicure Foods Corp., Elizabeth, N.J. - Training and Written Certification was not done for all employees operating Powered Industrial Equipment.  Condition noted on or about 3/14/2014.

The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  a- Epicure Foods Corporation, Elizabeth, NJ - The OSHA 300 log or equivalent was not maintained by the employer.
