29 CFR 1926.304(i)(1): Each circular hand-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut:    a)  An employee was using an unguarded Makita 10" Table Saw to cut and/or rip wood trims.    No abatement certification or documentation is required for this item.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) Employees were exposed to the hazard of falling while working from ladder jack scaffold platform elevated approximately 20 feet above the ground.     PM Windows & Doors Replacement, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(i), which was contained in OSHA inspection number 313658312, citation number 1, item number 4 and was affirmed as a final order on September 10, 2009, with respect to a workplace located at 653 N. Northwest Hwy., Park Ridge, IL 60062.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
