29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired.   a. In the administration office of Area C where employees were using a damaged extension cord to install windows, an inspection of the extension cord was not conducted prior to use, thereby exposing employees to an electrical contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. In the administration office where employees were installing windows, the strain relief was pulled away from the outer insulation of the extension cord, thereby exposing employees to a electrical contact hazard.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, including an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employees can obtain and use the appropriate hazard information.  a. At the worksite where employees used chemicals such as, but not limited to 795 silicone sealant, employees were not trained on safety data sheets, pictograms, or labels, thereby exposing employees to a contact hazard.
