29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. In the administration office of Area C where employees were using a damaged extension cord to install windows, an inspection of the extension cord was not conducted prior to use, thereby exposing employees to an electrical contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. In the administration office where employees were installing windows, the strain relief was pulled away from the outer insulation of the extension cord, thereby exposing employees to a electrical contact hazard.
