29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:      a)  Jobsite, 3846 Brownsville Road, Pittsburgh, PA 15227:  Observed on or about March 14, 2014, employees working on the roof of a two story residential dwelling were not protected from falling by the use of guardrails, safety net system, or personal fall arrest systems.  The slope of the roof was approximately 4 in 12 (vertical to horizontal).  The employees were exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.503(c):  The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:     a)  Jobsite, 3846 Brownsville Road, Pittsburgh, PA 15227:  Observed on or about March 14, 2014, the employer did not provide retraining to employees who were working on a 4 in 12 (vertical to horizontal) sloped roof while exposed to a fall hazard of approximately 20 feet to the ground below.  The employer did not retrain employees in the recognition of fall hazards and the procedures to be followed to reduce or eliminate these hazards.
