29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   a) Coolidge Avenue and South Street, South Plainfield, NJ 07080. One employee was observed operating a Makita circular saw cutting sheathing material while not wearing any eye and or face protection. A second employee was observed operating a Hitachi coil nailer securing sheathing material while not wearing any eye and or face protection. Both were exposed to eye and face injuries from flying debris.     Condition occurred on or about 03/11/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) Coolidge Avenue and South Street, South Plainfield, NJ 07080. Two employees were cutting and installing new sheathing materials while working on and from the roof with no fall protection systems in place exposing them to fall hazards.     Condition occurred on or about 03/11/14.
