29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a. On or about March 10, 2014, the employer did not ensure employees were protected from struck-by hazards when operating the Fanuc R-1000 robotic machine located in the robot programming room.  The machine was not guarded to protect employees who operated the machine in automatic mode to test programming.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized:    a.  On or about March 3, 2014, the employer did not develop and implement electrical safety related work practices which cover employees exposed to live electrical conductors while performing work.  Employees responsible for troubleshooting and/or performing electrical tests were exposed to electrical hazards in that safe work practices were not employed to prevent electrical shock and other electrical related hazards.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    a. On or about March 3, 2014, the employer did not assure that employees who perform electrical tests on live conductors use personal protective equipment to protect from electrical hazards.  An employee volt testing a 240 volt electrical test block was exposed to electrical shock and arc flash hazards in that insulated gloves with leather gloves and a hard hat were not worn during the volt testing activities.               b.  On or about March 3, 2014, the employer did not assure that employees who perform electrical tests on live conductors use personal protective equipment to protect from electrical hazards.  An employee, volt testing circuits and pulling/installing fuses in a 480 volt electrical panel was exposed to electrical hazards in that flame resistant clothing, face shield, and hard hat were not worn during the volt testing activities.            c. On or about March 10, 2014, the employer did not assure that employees who perform electrical tests on live conductors use personal protective equipment to protect from electrical hazards.  An employee, troubleshooting a 480 volt robot control system was exposed to electrical hazards in that a face shield and hard hat were not worn during the volt testing activities.
