29 CFR 1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the forks:    On or about March 13, 2104, employer did not ensure that a scaffold platform was attached to the forklift forks while employees erected a metal building. This condition exposed employees to fall hazards exceeding 10 feet.
