29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design: On or about March 13, 2014, employees worked in a scaffold platform that was not designed by a qualified person. The platform was supported by the forks, but the forks were not within the platform's fork pockets, nor was a locking pin used as specified by the Sky Trak manufacturer. The platform was secured to the forks by rubber ties and chains attached at the platform's guardrails. The excessive width caused the platform to be unstable.
