29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: (a) 170 Allen Boulevard, Farmingdale, NY (Worksite), Warehouse - The emergency exit door (Exit door # 2) located along the west side of the warehouse was locked with three different locks, including a door bolt slide lock, knob style lock, and a deadbolt lock while employees occupied and worked in the warehouse on, or about March 20, 2014. Note: Abatement for this violation was previously documented; therefore, the employer is not required to submit abatement certification or documentation for this violation in accordance with 29 CFR 1903.19.

29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: (a) (Worksite), Warehouse - The exit route (through Aisle # 2) leading to the emergency exit door (Exit door # 3), along the east side of the warehouse, was obstructed and blocked by the Raymond Forklift, Model # 537-CSR30T / Serial # 537-96-02142, while employees occupied and worked in the warehouse on, or about March 20, 2014. Note: Abatement for this violation was previously documented; therefore, the employer is not required to submit abatement certification or documentation for this violation in accordance with 29 CFR 1903.19.
