Jaz Industries, LLC, Virginia Marine Terminal, Norfolk Weld Shop: On or about 03/14/14, a welding lead was repaired within 3 feet of the electrode holder with black plastic electrical tape applied over exposed copper conductors, thereby exposed employee to a potential shock hazard.
