29 CFR 1926.25(a): During the course of construction, alteration, or repairs,  debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures:    On or about March 11, 2014, employer did not ensure the area was kept clean of construction material debris. This condition exposed employees protruding nails, trip and fall hazards.

Employees were exposed to fall hazards from and elevation exceeding 6 feet.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    On or about March 11, 2014, employer did not ensure the ladder was extending at least 3 feet over the roof's edge. This condition exposed employees to fall hazards greater than 6 feet.
