29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a,     On the west side of the second floor, there was an opening in the guardrail system where employees were framing walls thereby exposing employees to a 10'6" fall hazard.  b.     On the southwest side of the second floor where framers were unloading materials, the guardrails were removed and not replaced thereby exposing employees to a 10'6" fall hazard.

On the first floor, there were several floor holes varying in size from 23 inches across to 11 inches deep thereby exposing employees to a trip/fall hazard.

On the site where chemicals were used, the employer did not provide training under the new hazard communication program for labeling and safety data sheets thereby exposing employees to a contact hazard.
