The violation occurred in the Silver Creek Processing Facility on or about March 19, 2014 and at times prior thereto and after where the employer failed to ensure that an engineering evaluation of the inlet pressure drop that exceeded 3% of the relief device set pressure had been conducted in accordance with recognized and generally accepted good engineering practice such as, API STD 520 for relief device(s) such as but not limited to:        a.     PSV 501 (PCV 402B Failure, Inlet Pressure Drop at required rate > 7.9%)   b.     PSV 464 (External Fire, Inlet Pressure Drop at required rate > 10.0%)   c.     PSV 402 (External Fire, Inlet Pressure Drop at required rate > 3.5%)   d.     PSV 404 (External Fire, Inlet Pressure Drop at required rate > 7.1%)      Pursuant to 29 C.F.R. 1903.19, within (10) calendar days of the abatement date the employer must submit documentation showing that it is in compliance with the standard, including the description of the steps that it has taken to ensure that equipment complies with recognized and generally accepted good engineering practices.
