29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit:  (a) Silo, 2020 Mill Lane, Williamsport, PA - Employees were assigned to enter and entered the silo (designated by the employer as a permit required confined space) to remove wood and perform repairs without the preparation of an entry permit that documented the required safe entry procedures. Employees were exposed to atmospheric, caught under and struck-by hazards, on or about February 21, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage:  a) Grading Station, 2020 Mill Lane, Williamsport, PA - Employees worked underneath a light fixture used to provide general illumination that contained two (2) linear fluorescent bulbs that were not guarded thereby exposing the bulbs to breakage and damage. Employees were exposed to a struck-by hazard, as observed on or about March 20, 2014. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a) Grading Station, 2020 Mill Lane, Williamsport, PA - Employees walked on and over a flexible cord that supplied electrical power to the Grading Station control panel. Employees were exposed to contact with an electrical current hazard, as observed on or about March 19, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.  b) Grading Station, 2020 Mill Lane, Williamsport, PA - Employees walked on and over a flexible cord (extension cord hard wired into the Grading Station control panel) that supplied electrical power to the Grading Station portable remote controller box. Employees were exposed to contact with an electrical current hazard, as observed on or about March 19, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  a) Grading Station, 2020 Mill Lane, Williamsport, PA - Employees used the Grading Station control panel to operate the Grading Station equipment with exposed conductors at the joint where electrical power entered the Grading Station control panel. Employees were exposed to contact with an electrical current hazard, as observed on or about March 19, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is not required for this violation. Abatement was observed during the inspection.  b) Grading Station, 2020 Mill Lane, Williamsport, PA - Employees used the Grading Station portable remote controller box to operate the Grading Station equipment with exposed conductors at the joint where electrical power entered the Grading Station portable remote controller box. Employees were exposed to contact with an electrical current hazard, as observed on or about March 19, 2014.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  a) Boiler Chemical Storage Area, 2020 Mill Lane, Williamsport, PA - The portable fire extinguisher mounted near the chemicals for the boiler had not been visually inspected at least monthly since July, 2013. The portable fire extinguisher was not charged and exposed employees who choose to attempt to extinguish a fire with the portable fire extinguisher to fire hazards, on or about March 19, 2014.    In accordance with 29 CFR 1903.19(c) and (d), abatement certification and documentation is required for this violation.      The Eastern Wood Products Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315799411, citation number 3, item number 3 and was affirmed as a final order on May 14, 2012, with respect to a workplace located at 2020 Mill Lane, Williamsport, PA.
